Liability Waiver & Release Form

1. Release of Liability

Participant fully releases, discharges, and absolves Enso Studio™, its owners, instructors, affiliates, and agents, from any and all liability for injury, accident, or damages that may occur during participation in any fitness, dance, or wellness class at 10000 Coors Blvd NW, Albuquerque, NM 87114.

2. Assumption of Risk

Participant understands and voluntarily assumes the risks associated with physical activity, including the risk of accident, injury, or illness. Participant accepts full financial responsibility for any costs related to such incidents.

3. Medical Clearance

Participant affirms that they have consulted with a physician and are medically cleared to participate in fitness, dance, and wellness classes.

4. Medical Responsibility

Enso Studio™ does not assume responsibility for medical expenses, injuries, or damages incurred during participation in its classes, services, or use of its facilities.

5. Communication & Privacy

Participant agrees that their email address may be used solely for class-related communication, including cancellations, rescheduling, promotions, and studio updates. Personal information will not be shared with third parties.

6. Media Release (Optional)

Participant understands that photography or video recording may occur during classes. By signing below, Participant grants Enso Studio™ permission to use their image or likeness for promotional purposes.
